What Happens During a Whiplash Injury?

Whiplash occurs in milliseconds. During a rear-end collision, for example, the torso is accelerated forward while the head momentarily stays behind — creating a violent hyperextension of the cervical spine. Then the head snaps forward into hyperflexion. This two-phase motion happens faster than any muscle can react to protect the joints, ligaments, and discs of the neck.

The result is a spectrum of injuries: micro-tears in the cervical muscles and ligaments, joint capsule injuries at the facet joints, potential disc herniations, and damage to the soft tissue structures that normally provide cervical stability. The nervous system's protective response — muscle guarding and spasm — adds a second layer of pain on top of the original injury.

What makes whiplash particularly important to treat properly is what happens if it's left alone. Scar tissue that forms in damaged soft tissue creates adhesions — restricted, fibrotic tissue that remains painful and limits motion long after the original injury should have healed. This is the primary driver of chronic whiplash, and it's exactly what Active Release Technique is designed to address.

    Symptoms of Whiplash

Whiplash produces a wide range of symptoms, many of which don't appear until 24–72 hours after the injury. Don't assume you're fine because you felt okay immediately after the accident.

Neck pain and stiffness, often worsening on day 2–3 post-injury
Reduced cervical range of motion — difficulty turning head
Headaches, typically starting at the base of the skull
Shoulder, upper back, or between-shoulder-blade pain
Arm pain, tingling, or numbness (nerve involvement)
Jaw pain or difficulty chewing (TMJ involvement)
    Dizziness or balance problems
Difficulty concentrating, memory problems ('whiplash brain')
    Fatigue, irritability, sleep disturbances
Blurred vision or ringing in the ears

Important: Cognitive symptoms — difficulty concentrating, memory problems, irritability — are common and real after whiplash. They stem from the disruption to cervical proprioception and can affect work performance and quality of life. These symptoms respond well to proper chiropractic care.

    Common Causes of Whiplash

    Rear-End Car Collisions

The most common cause. Even low-speed impacts (5–10 mph) generate enough force to injure cervical soft tissues. The head snaps backward then forward in milliseconds — faster than the muscles can react to protect the spine.

    Sports Collisions

Football tackles, hockey checks, soccer headers, and martial arts can all produce whiplash-type injuries. Contact sports and high-speed activities carry significant risk.

    Falls

Falling backward and striking the head, or falling and catching oneself with outstretched arms, can transmit enough force to the cervical spine to cause whiplash.

    Physical Assault

Being struck in the head or shaken vigorously (as in assault or abuse) can cause whiplash-equivalent injuries to the cervical soft tissues.

    Amusement Rides

High-acceleration rides, particularly roller coasters, generate rapid cervical flexion-extension that can injure cervical ligaments, especially in individuals with pre-existing disc degeneration.

    Work Accidents

Falling objects, slips, and industrial accidents can produce whiplash mechanisms outside of vehicular accidents.

    How Dr. Haynes Treats Whiplash

Whiplash is primarily a soft tissue injury — which is why Dr. Haynes's ART certification is especially valuable. Treatment addresses both the joint injuries and the soft tissue damage simultaneously.

    Gentle Cervical Adjustments

Carefully calibrated cervical mobilization and manipulation restore joint motion, reduce protective muscle spasm, and address the joint injuries that are central to whiplash.

    Active Release Technique (ART)

ART is exceptional for whiplash because it directly addresses the soft tissue damage — the torn muscle fibers, ligament injuries, and adhesions that form during healing — that standard adjustments alone cannot fully resolve.

    Soft Tissue Therapy

Targeted work to the suboccipital muscles, cervical paraspinals, scalenes, and upper trapezius reduces the chronic muscle guarding patterns that perpetuate whiplash pain.

    Cervical Rehabilitation

Progressive strengthening of the deep cervical flexors and stabilizers restores the neuromuscular control that is disrupted by whiplash, preventing chronic pain and re-injury.

    Auto Accident Documentation

For patients injured in car accidents, Dr. Haynes provides thorough documentation of your injuries, treatment, and progress — which is essential for insurance claims and any legal proceedings. We work with personal injury attorneys and handle insurance billing directly. Call us immediately after your accident: (312) 658-0658.

    Whiplash Recovery Timelines

Recovery depends on the grade of injury, how quickly you started treatment, and whether there is disc involvement. Here is what most patients experience:

    ConditionExpected TimelineWhat to Expect
Grade 1 whiplash (pain/stiffness only)4–6 weeksMost Grade 1 cases resolve fully with prompt chiropractic care and movement-based rehabilitation.
Grade 2 whiplash (reduced ROM)6–12 weeksRestoring full cervical range of motion takes longer, but most patients return to normal function within 3 months.
Grade 3 whiplash (neurological signs)3–6 monthsNerve symptoms require additional care. Most resolve with conservative treatment, though timeline extends with severity.
Whiplash with disc herniation8–16 weeksCervical disc herniations from whiplash require disc-specific techniques alongside soft tissue care.
Chronic whiplash (untreated)3–6 monthsMature scar tissue takes significantly longer to resolve than fresh injury — early treatment is always better.

    Self-Care After a Whiplash Injury

What you do in the first days and weeks after a whiplash injury significantly impacts your recovery trajectory. These strategies support healing between appointments.

    Stay gently active

Avoid complete rest — gentle movement maintains circulation and prevents the stiffness that worsens chronic whiplash. Short, gentle walks and light daily activities are appropriate from day one.

Ice in the first 72 hours

Apply ice wrapped in a cloth for 15–20 minutes every 1–2 hours during the first 48–72 hours. Ice reduces acute inflammation and muscle spasm. Switch to moist heat after the acute phase.

    Sleep with a cervical pillow

Use a pillow that supports the natural cervical curve — neither too flat nor too high. Avoid sleeping on your stomach, which rotates the cervical spine and strains injured structures.

    Avoid phone posture

Looking down at your phone puts 40–60 lbs of stress on the cervical spine. Hold your phone at eye level and take frequent breaks. 'Tech neck' dramatically slows whiplash recovery.

    Gentle range-of-motion exercises

Once acute inflammation subsides (3–5 days post-injury), gentle neck rotations and side bends performed slowly within a pain-free range help restore mobility and prevent scar tissue adhesions.

    Document everything

If your whiplash was from a car accident, keep a symptom diary. Note pain levels, activities affected, and symptoms daily. This documentation is critical for insurance claims and personal injury cases.

Why Choose Comprehensive Chiropractic for Whiplash?

    ART-Certified for Soft Tissue

Whiplash is a soft tissue injury first. ART directly addresses the muscle tears, ligament injuries, and adhesions that drive chronic whiplash pain.
